Support Worker (Community)

Exciting new opportunities at Carer Support Wiltshire Following the award of a large contract to support carers of all ages in Wiltshire over the next five years, a number of exciting new opportunities have arisen to join this forward-thinking energetic team, committed to supporting carers in Wiltshire and beyond.

With a large volunteer workforce, CSW are leaders in the local voluntary, community and social enterprise sector (VCS). Their work is recognised nationally making this a busy and fulfilling place to work.

Position: Community Connector - Carers
Location: Wiltshire, flexibly based in one of six geographical areas
Job Type: Part Time, Permanent
Hours: 18.5 hours per week Monday to Friday plus occasional weekend / evening
Salary: £21,530 to £23,698 per annum pro rata

About the role:

As the Community Connector you will ensure unpaid carers of all ages within the community are provided with the right information, advice and support at the right time. Work in designated local area using an asset-based model, to deliver service in partnership with statutory, voluntary and community-based organisations and volunteers, including the public.


- Make initial contact with carers, undertake a proportionate assessment or offer the appropriate information and advice based on appearance of need. Refer or signpost according to the carer’s wishes and needs
- Organise and run regular carer clinics in a range of venues including GP surgeries
- Provide one to one support to carers following time-limited, asset-based approach
- Facilitate group support depending on identified local need
- Adhere to CSW’s safeguarding policies and procedures to ensure carers and the people they care for are able to live in safety, free from abuse and neglect
- Prioritise cases on personal caseload, taking into account the needs of the carer, areas of expertise required (if complex), ensuring that carer has access to relevant community services and support
- Ensure that any young carers identified are referred for assessment to the local authority and given appropriate information and advice at first point of contact
- Work in collaboration with local services including the public and unpaid carers to develop resources to meet the needs of carers in each geographical area
- Identifying appropriate sources of information, advice, benefits, equipment and services which carers can access, and updating shared resource information to reflect this
- A flexible approach is required for this role and you may be asked to undertake other duties from time to time as determined by your line manager and in keeping with the purpose of the post and after consultation with the post holder

About you:

- Good standard of education
- Further qualifications in health and social care. inc, linked NVQs, social work degree, nursing qualification or equivalent experience is desirable
- Experience of carrying out assessments over the phone, and face to face
- Previous experience working within a community and developing professional relationships in that area
- Understanding of all age carers’ issues and rights
- Good understanding of the Care Act 2014, Children and Families Act 2014, the Children’s Act 2004 and other linked legislation
- Knowledge of multiagency working, including: whole family assessments, holistic approaches and liaising with other agencies
- Knowledge of person-centred planning and working with service users to agree asset-based support would be advantageous
- Excellent verbal and written communication skills
- Trust and respect, Working together, Creativity, Responsibility qualifications Level 2
- Leadership qualification level 1
- Able to work flexibly in terms of location and occasional evening and weekend work
- Car driver and owner
