Case Manager - Children's Team
I have a very exciting opportunity for a registered Health and Social Care professional to join a well-established organisation, as a Case Manager.
The organisation offers services to their Clients throughout the North West and North Wales who have suffered catastrophic injury and specialise in working with Clients following acquired brain and spinal injury and other complex disabilities.
They also have a bespoke Children and Young People's team who offer services to younger people with catastrophic injury or illness, or who have birth injuries such as cerebral palsy. This vacancy is sepcifically to work in this team
This position is open to Nurses, Paediatric Nurses, Occupational Therapists, Physiotherapists and Social Workers - with peadiatric qualifications and experience.
As a Case Manager, you'll assess organise and manage bespoke care and rehabilitation packages for Clients who have acquired a brain injury, spinal injury or life-changing physical disabilities usually due to an accident or a birth injury.
Full, extensive training will be provided from day one.
Salary - Starting salary is in the region of £30,000.00 depending on experience.
On successful completion of induction stake holder pension currently paid at 3% plus Medicash
Hours - 35-hour week, Monday - Friday 9-5.
This organisation supports clients throughout the North West.
You will be responsible for completing a thorough and detailed assessment of a Client's needs, taking a holistic approach.
You will identify their rehabilitation, care, emotional and social needs.
You'll implement a rehabilitation and appropriate care package, if and when required, and continually review and assess it, to ensure that it continues to meet the client's goals.
The care package will be delivered by an external provider or through supporting the client to directly recruit appropriate staff and to ensure that the care plan is followed.
Most clients are involved in a claim for personal injury and you will support their claim through accurate report writing and continual re-assessment.
You will be able to put into place bespoke packages of care, therapy and equipment, vehicles, etc. that will benefit the client's heath and emotional needs and rehabilitation.
You'll work within a Multi-disciplinary team consisting of Nurses, OTs, Physios, Social Workers and other Health and Social Care professionals and be able to share knowledge and experiences with others within the organisation.
If you are an experienced and active Health and Social Care professional within Children's services who has an interest in Case Management, you can apply, providing you have the below:
- You will need a minimum of 2 years post qualification experience.
- Peadiatric qualification/experience
- You will need to be a driver with your own vehicle - mileage is paid at 45p a mile.
- Ideally you will have worked in the community before and/or have previous experience of working with young people following Brain Injury.
