The team is a social work service based within the hospital, working with patients and their families and carers, who are admitted to hospital. Our tasks are to assess and plan and support service users (SU's) to cope with social, emotional, economic and environmental problems. We also help SU's to maintain their independence and to live as normal a life as possible in the community and to work jointly with health staff on social matters that may affect present and future health. We will often draw upon domiciliary, day and residential services to support SU's on discharge from hospital.

For this position you must also have:

•a degree or masters in Social Work (or any other recognised equivalent qualification)
•2+ years' experience in Children and Families Social Work.
•HCPC registration
•Be able to commit to working some weekends.
