You will be Assessing, planning and implementing specialist strategies to meet individual needs, which will include long and short term care planning, and medication management where suitable.The team of experienced hand picked staff work with the service user and their family to create a person centred plan of care and support to meet their needs.The team here believe that coming to live here is the start of something special, with a unified aim of helping people reach their full potential regardless of illness or disability.
