The Hospital Care Coordinator enables Bupa UK Customer to improve member outcomes and experience. The role focusses on ensuring that Bupa inpatients admitted to selected London Hospitals are cared for at the appropriate level of care and for the clinically appropriate length of stay. The Hospital Care Coordinator also supports the hospital with timely, efficient and sustainable discharge of the member.
The role requires utilisation of excellent clinical skills and experience in an acute setting. You will be supported by a clinically based and internationally recognised software package when reviewing Bupa inpatients. . You will make decisions around the clinically appropriate levels of care and length of stay following accurate review of clinical records, consideration of the software outcomeand engagement with the relevant care providers, including Clinicians.

Accountabilities:
Confirming Bupa inpatient admissions and discharges, and prioritising reviews on a daily basisIdentifying and seeking relevant clinical information to accurately enter into selected clinical softwareLiaising with other Bupa teams to ensure best outcomes for the memberMaintaining basic reporting tools Identifying where Bupa inpatients level of care, care setting or length of stay does not meet criteria.Analysing the outcomes from the software and determining if further information or action is requiredBuilding and maintaining excellent working relationships with Hospital personnel Facilitating an excellent experience for Bupa inpatients Facilitating communication and resolution of issues/ concerns between hospital staff and Bupa customers
Requirements:
Excellent interpersonal and communication skills is paramount to this roleRegistered RGN with at least 5 years post qualification experience on a ward and/or within other clinical areasBed management and discharge planning experience highly desirableExperience with decision making in a clinical setting Strong analytical skills are essentialYou will have experience across a broad range of clinical specialtiesKnowledge of the treatment pathways specific to service user requirementsGood IT skills; ability to navigate around Microsoft Office, and comfortable working with computers Excellent organisational skillsDemonstrated ability to communicate with both clinical and non-clinical personnel across all levels of an organizationWorking knowledge of healthcare system and resourcesExperience working independently and as a team member
