The Horizons Service operates in five localities across Kent to provide intensive rehabilitation and recovery focussed support to service users in their own homes. The team use an holistic approach together with partners from 3rd Sector agencies who provide additional Social Care and landlord/tenancy support in order to enable individuals with complex mental health needs to manage more effectively in the community.
We currently have an exciting opportunity for an enthusiastic and motivated individual to develop the role of team leader within an established team in Thanet The successful applicant will care coordinate and manage a defined caseload of individuals with complex mental health problems. The post also involves day to day management and supervision of support workers within the team in order to deliver comprehensive, individualised treatment and recovery plans which include supporting clients to maintain their living environment, and encouraging and supporting them to have meaningful roles and purpose within their local communities.
The post involves lone working, 1 to 1 work and group-work and individualised programmes of care in order to support clients to develop and enhance their skills and independent and promote recovery. The successful candidate will be expected to work closely with partners in the scheme, to advocate for clients and to promote a collaborative and recovery orientated focus throughout. They will also be expected to work flexible hours between 08:00 and 20:00 according to the needs of the service, this will include some, evenings, weekends and public holidays.
The post is open to Registered Mental Health Nurses and Occupational Therapists with some post registration experience. So if you have previous experience in working with individuals with mental health problems in a community setting are flexible and resourceful, have excellent verbal and written communication skills and leadership experience we would be very interested in hearing from you.
The ability to travel in a timely manner between locations is essential for this post.
