Exciting New service looking for experienced cancer and/ or palliative nurses. Are you an experienced cancer and / or palliative care nurse looking for a new challenge or looking to develop your nursing career in a new direction?A unique opportunity has arisen for two suitably qualified senior nurses to join our My Care My Way team and to play a key role in the development of multidisciplinary care, with a cancer and palliative care focus, within our newly established Integrated Care Centres.My Care My Way is a new Integrated Care service designed to support the health and wellbeing of local people aged 65 and over. The main focus of the service is to allow patients to have access to a comprehensive multidisciplinary team input. The team comprising of health professionals from both health and social care backgrounds will offer comprehensive assessment of care needs, develop a bespoke care plan and allow the patient to be cared for as close to home as possible for as long as possible. The role has two key aspects a) to promote wellness through comprehensive cancer reviews and influence patients to participate in the uptake of services to maintain their health and b) to identify patients early with progressive disease and ensure that advance care plans may be developed and supportive services are utilised as appropriate.We are looking for a dynamic person with experience of working across a range of health care settings, developing innovative services, as this is a new service there will be an expectation that you will engage in collating data for evaluation and service review. These roles will be hosted by Central London Community Healthcare NHS Trust (CLCH) and supported by Macmillan Cancer Support. As a Macmillan post holder you will have access to a range of services and opportunities to support your professional development.Central London Community Healthcare (CLCH) working in partnership with Macmillan Cancer Support and NHS West London CCG, are looking to recruit Band 7 palliative care nurses to work in a new model of integrated care designed and developed in collaboration with our patients and local health and social care partners. This is an exciting opportunity for experienced nurses with a background in cancer or palliative care to utilise their experience in these newly created roles. We need creative thinkers who understand how to adapt to new situations and have a passion to work differently in the community.You will be working as part of the My Care My Way Team. The team will work collaboratively with patients and their carers to ensure that patients receive the necessary support to understand and manage their condition themselves, wherever possible, with a clear treatment plan and information on the range of services available and how to access them. In addition to your clinical nursing skills you will also have the opportunity participate in service development and be involved in teaching and training for My Care My way staff, as well as local GPs and other local staff health and social care partners. This will be an initial two year fixed term contract/ Secondment opportunity.
