UCLH has two major strategic projects underway. On Grafton Way, we are building a facility which will be home to one of only two high energy NHS proton beam therapy centres, Europe’s largest centre for the treatment of blood disorders and a hyper modern surgery service. Close by on Huntley Street we are developing the new Royal National ENT and Eastman Dental Hospitals. Replacing the current hospitals on Grays’ Inn Road, this development will mean we can provide multidisciplinary care to our dental and ENT patients in a modern environment. Our Grafton Way facility is due to open in 2020, with Huntley Street opening in 2019.

UCLH has a vacancy for the above post in the Division of Integrated Care working as part of the Integrated Discharge Service (IDS). The Integrated Discharge team is led by the Senior Nurse with management and support from four Band 7 specialist Sisters covering the services of Simple and Complex Discharge, Homeless support, Admissions Avoidance in the Emergency department (7 days) and Alcohol and Drugs specialist service. The team also work in the Emergency Department and Acute Medical Unit each day with patients and by working with our community colleagues support the patients discharge and prevent readmissions.The team work seven days a week from 08.00 to 20.00 (9-5 at weekends) facilitating and supporting patient discharges.In order to be successful in the role, the applicants must have a keen interest in improving the patient experience on the ward, and care about the continuity of the service between the acute and community setting. You will have experience of working within a discharge planning service and a good knowledge and experience of community services and how to ensure safe and timely discharge and prevent readmissions. The ward support for discharge will involve leading and contributing to the co-ordination of patients’ discharge arrangements and assist in the administration processes relevant to discharging patients from UCLH. The team work with the ward multi-disciplinary teams to prevent potential delays to discharge such as complex functional, social and home environment needs. This includes telephone follow up of patients after discharge. The successful candidate will be a dynamic and motivated nurse , who can provide decisive and effective input and who has effective communication skills, a flexible approach and an ability to work under pressure, both as part of a team and independently
