Band 5 Clinical Practitioner (Nurse)Camden and Islington NHS Foundation Trust has an established multidisciplinary home treatment team to support people with dementia and older people with acute metal illness in their own environment. The service also gate-keeps all admissions for people from the age of 70 to Highgate Mental Health Centre, and works closely with several acute hospitals and the liaison services. The service is looking for a Band 5 nurse who is passionate about working with older people and those who have dementia. This is a service and an ideal opportunity for an enthusiastic nurse who wants to work in a new team and influence how care is delivered for this client group. The post holder will work as an independent practitioner but within a supportive team to deliver care that enables service users to remain in their own homes. The post holder will work with the team to maximize the service users’ ability to cope with their mental health crisis in their own environment and regain their independence. The post holder will deliver evidence based care that is in line with NICE guidelines and best practice.
