This post is part of an exciting expansion of CAMHS specialist services within HPFT, supported by NHS England’s innovative ‘Testing New Care Models in tertiary mental health services’ programme (Wave 2). Our new model of care for Tertiary Services has been designed to transform local service provision in Hertfordshire by integrating the Tier 4 and Tier 3 CAMHS pathways, to ensure that young people across the county can access the services that meet their needs at the right time. The new Home Treatment Team (HTT) will build on the existing CAMHS Crisis Assessment and Treatment Team (C-CATT) model and high risk pathway, to prevent avoidable admissions and support discharges from inpatient units, so that young people are in the least restrictive service for their need. The Trust is looking for a range of experienced and dynamic professionals to join the HTT team, to deliver a treatment approach which enhances choice for young people, takes a positive risk management approach and offers an integrated case management approach for young people across Tier 3 and Tier 4 services. This role will involve providing treatment to young people who are at high risk but can be managed safely at home, with the right support from an expert clinical team.
