Mental Health Link Practitioners (Band 6) for General Practice North Ayrshire

An exciting new opportunity has arisen for 4 Mental Health Link Practitioners (Band 6) for General Practice working as part of North Ayrshire Health and Social Care Partnership's Adult Community Mental Health Service.

These posts will initially scope the requirements for population of the General Practices that they are attached to. This initial period will require the development of data analysis methodology review and evaluation. Following completion of this scoping period the findings will be implemented as a test of change in conjunction with the GP practice and General Practice Multi disciplinary Teams Mental Health Services and Team Leader.

You will have a registered professional qualification with the NMC SSSC or HPC and have experience in mental health assessment and care planning with an interest in working with patients with mental health problems in the community and General Practice setting.

Evidence of post registration education. Advanced communication skills well developed risk assessment skills and excellent organisational skills are essential together with evidence of continued professional development.

The candidate may be responsible for a caseload of service users working in partnership with the General Practice team health and social care partnership services service users and voluntary groups to support and underpin a seamless journey through the health and social care system to maintain consistently high standards of service delivery and care in a climate of change and redesign.

These posts are a fixed term/secondment opportunity through until 31st March 2019.

Car owner/driver is essential.

Hours: 37.5 per week over 5 days Monday-Friday Evening Clinics may be required within the scope of this post.
