Our client owns and provides care services for two care homes in the Stepney area of Tower Hamlets.

They are the largest provider of care homes for older people in the London Borough of Tower Hamlets, owning 2 of the boroughs 6 care homes.

Their care homes are for people who require 24 hour care and support. Their homes are comfortable, homely and inviting where highly trained staff are on site 24 hours a day to meet the everyday needs of the tenants.

To provide a high quality of personalised care and support to elderly residents of our clients care homes, working within a staff team, who provide a 24 hour service 365 days of the year. Ensure full compliance with CQC regulatory standards and our clients policies and procedures

Providing person centred care in line with resident’s care plans to ensure that all physical, mental, social, personal relationships, emotional and cultural needs are being met.

Key working, including co-producing care plans and risk management plans with residents.

Maintaining up to date daily records and sharing relevant information about residents and their changing needs and circumstances during the course of the day, at handover meetings and in team meetings as appropriate.

Ensuring that residents are fully involved with all choices about every aspect of their care.

Undertaking the safe administration of medication/homely remedies.

Helping residents with daily personal care such as washing, dressing, using the toilet and feeding themselves

Helping with mobility issues and other physical disabilities, supporting people to use aids and personal equipment as appropriate.

Liaising with other health and social care professionals as part of the care planning process in order to provide holistic care

Supporting people with end of life care
Supporting people living with Dementia
Supporting people to access a range of services from other agencies: e.g. faith and community groups, advocacy and transport services, emotional and bereavement services

Skills

Ability to relate to people from a wide variety of backgrounds
Good level of Numeracy & literacy
Computer skills - word/outlook
Efficient administrative skills

Qualifications

QCF Level 2 Diploma in Health and Social Care or The Care Certificate
Certificate in First Aid
certificate in Medication administration within 3 years

Working with people with Dementia
At least 12 months experience of delivering personal care
Working with older people
Working with people with dementia
Working with older people from diverse background/community
Collaborative working
Working across both homes
Working with residents and staff to organise activities in the homes
Flexible working and working on a shift pattern to a planned rota
