Floating Support Worker
Responsible for: Providing advice, guidance and support vulnerable young people aged 18-24 years. This role may include working with those in crisis, and / or those at risk of immediate homelessness. Job purpose: To manage an effective and successful sub-contract and delivery of a floating support contract across Hertfordshire. Floating support assists vulnerable young people to set up and or / sustain their tenancies thus helping to prevent repeat homelessness. You will provide housing advice and support, including information on the relevant benefits and will assist them to identify other support needs, including mental health issues, offending behaviour, issues around family breakdown, neighbourhood disputes, behavioural problems, anti-social behaviour and parenting support. You will help co-ordinate a solution focused action plan that involves other agencies as needed and encourages the young person to take responsibility for all areas of their lives and promote healthy, successful outcomes that are holistic, yet realistic. Specific Duties: Manage a varied, short term case load with conflicting deadlines To work in partnership with the Aldwyck Floating Support staff to provide a consistent service countywide. To accept referrals from the Senior Floating Support Worker To ensure that all young people work towards an agreed support plan which is regularly reviewed and adapted to ensure that they meet their individual needs and goals To ensure service users are fully aware of their rights and responsibilities To advocate on behalf of young people where appropriate To promote and facilitate Service User participation across the service To implement identified improvements in consultation with the Director of Operations, Floating Support Manager and Senior Floating Support worker To work as part of a team in the planning, organising and reviewing of the support service in consultation with Director of Operations and Floating Support Manager To assist with other hyh services cover where required Develop and maintain smooth, accurate pathways on the database for gathering and maintaining all relevant service user and statistical information as required accurately and within the specified deadline Offer realistic advice to young people, family members and professionals to prevent homelessness wherever possible. Identify any other needs of the young person / family members and refer into appropriate services / co-ordinate support as required Report any safeguarding (child protection) concerns to your line-manager and refer as appropriate To carry out follow-up work with young people, families and professionals Develop and share knowledge and good practice To establish and maintain positive working relationships with statutory and non-statutory agencies Ensure hyh policies and procedures are adhered to at all times Work alongside volunteers / students as appropriate and support with training Identify personal training needs in agreement with the line manager and attend training as appropriate To attend supervision, team meetings and other meetings as appropriate To work as part of a team communicating with other staff where appropriate Duties in common with other members of Team: To be an ambassador for the charity both internally & externally in order to further hyh aims whilst protecting and enhancing its reputation To report accidents and record incidents accordingly To maintain the best appearance of your immediate working area To undertake any other reasonable duty that may be required from time to time Closing date: 9am on Monday 29th January 2018 Recruitment day: Wednesday 31st January 2018 Personal interviews: Friday 2nd February 2018 Please note we do not accept CV's, if you wish to apply please visit our website and download the short application form.
