The East Edinburgh GP Cluster has successfully bid for funding from Focus on Dementia to become one of 3 national innovation sites for delivering effective and sustainable post diagnostic support within a primary care setting for people recently diagnosed with dementia or cognitive impairment. This work is closely linked to delivery of Scotland's National Dementia Strategy 2017-2020.
We are now looking to recruit a Dementia Support Facilitator in order to help us deliver this ambitious and exciting new service.
This a Fixed Term post until the end of March 2019 in the first instance.
If you are an enthusiastic motivated individual who is experienced passionate and committed towards delivering services to people living with dementia and their families then we would like to hear from you. You will work with patients registered across a number of GP practices in the local area.
You will be organised have excellent communication problem solving and decision making skills. You will have experience working with people living with dementia and providing person centred support. You will be able to deliver care at a one-to-one level but also in groups and liaise with a range of organisations in order to implement personalised plans for the individual and their family.
You will have a recognised professional qualification in e.g. nursing social work occupational therapy community work counselling or other related occupations to a minimum of SVQ Level 3 or equivalent.
