To organise and supervise the delivery of complex home care services to people with medium to long-term complex care needs, living in their own homes. Work in a flexible and reliable manner in response to the needs of the service. To co-ordinate the set up process and on-going requirements for support workers.
You will be accountable for the recruitment process for complex care workers, ensuring candidates are recruited in line with CQC standards.
You will support complex care workers to maintain safe working practices to client’s in line with the National Minimum Standards for Care.
You will manage the referral process. Undertake initial assessment visits where needed to clients to establish the package of care needed
You are responsible to deliver to each complex care worker a full induction into Bespoke Health & Social care and the package of care they are assigned to
You maybe called upon to visit a potential new client to establish needs and positively promote Bespoke Health & Social Care when required.
You are responsible in ensuring monthly rotas for complex care workers are completed in a timely manner and ensure all clients receive service as designated in their care plans.
You are responsible for managing sickness, absence and annual leave effectively so this has no negative impact on the client or BHSC
You must hold regular reviews of the service to ensure that care delivery reflects the clients needs and ensures that the client and/or their representative are fully involved.
You must hold regular team meetings, spot checks, supervisions and appraisals on all complex care staff ensuring quality of care is maintained at all times
You must attend monthly BHSC national meetings and other meetings where deemed necessary
Responsible for information and documents on each client and data input on BHSC IT software.
Carries out supervisions and appraisals for care staff.
Complete all risk assessments necessary and other health & safety documents prior to are being carried out correctly and that records are maintained and updated as required.
You may be called upon to assist clients with any other specified tasks as directed by the line manager, support services, relatives/representatives.
You are responsible for keeping accurate and relevant records on Bespoke Health & Social care’s IT systems
You will participate in on-call rota as directed by the line manager.
In liaison with the Bespoke Health & Social care clinical team, you must communicate any concerns about the client’s health and welfare
You must ensure a clinical incidence form is completed and sent to the Clinical Director when any accidents, incidents or complaints have been raised, observed or received
You must keep your line manager informed of any difficulties/changes in personal circumstances.
In consultation with your line manager identifies areas requiring personal development and undertakes training as required.
Highlight and or attend supervisory and training sessions as directed by line manager.
You are responsible for carrying out all duties in line with Bespoke Health & Social Care Policies and Procedures and legislative requirements.
At all times promote, respect and uphold the dignity of client’s. Participate in and actively support activities to promote the dignity of client’s, their families and carers.
You are responsible for your own Health and Safety, ensuring a safe working environment for your complex care workers and colleagues.
You are responsible for completing an Environmental risk assessment on all client’s to ensure there is a safe working environment for you complex care workers
Undertakes any other duties in line with the basic objectives of the post.
You are responsible for effective communication with client’s, colleagues CCG’s and Social Services. Writing monthly reports to the CCG, attending weekly conference calls with line manager and colleagues and being visual and available to clients at all times
