Role: Therapy Coordinator

Active Care Learning Disability model
People’s care and therapeutic goals are supported by our pioneering Active CareTM Learning Disability model. Developed by our in-house multi-disciplinary team, Active CareTM Learning Disability model. facilitates step-by-step objectives that cross all areas of an individuals’ psychological and behavioural development. Importantly, this includes substantial Speech and Language Therapy inputs and the use of Assistive Communication Technology enabling improved communication and social skills.
The emphasis on the role is to effectively deliver planned activities for patients under the direction of Occupational Therapy department and Multi-Disciplinary Team. You will be expected to contribute ideas, create and facilitate events & activities safely and support people to structure their time through meaningful activity. This will take place in the hospital and community setting with hours to be agreed in interview.
The successful candidate will be required to implement individual care plans during activity sessions and take into account specific interests and opportunities to promote choice and independence.

We are looking for a dynamic individual to work with our patients promoting positive environments to assist in the rehabilitation process. Our goal is to build the living skills, insight, education and confidence required to assist our patients to return to the community.
Joining a company with a growing reputation for delivering a contemporary approach in the field of intensive rehabilitation, you will be encouraged to contribute to, and influence the development of active care for our patients as part of the MDT. You will devise and implement activities appropriate to patients’ needs and requests.
Have basic computer skills (report writing)
Previous experience/skills in care setting are desirable.
Enthusiastic and motivated and communicate easily with all groups of people.
You must demonstrate the ability to manage time efficiently and autonomously on a day to day basis reflecting the dynamic and fast paced ward environment. Knowledge and understanding of the importance of risk assessment and relational security is essential as is experience and confidence in lone working. Must be proficient at note writing, documentation and IT literate.
