An opportunity has arisen for an Intermediate Care Support Manager within a new and evolving FSHC Intermediate Care Service (ICS). Reporting directly into the Intermediate Care Clinical Operations Manager (UK) this is a rare chance to join an award winning business that are at the forefront of innovation and investing significantly in new models of care within the health and social care market. The Intermediate Care Support Manager is responsible for leading and supervising the excellent standards of care to patients and their families, ensuring the effective providion of nursing care for patients requiring rehabilitation. To be successful you will need to have:
• An AHP or Nursing qualification and be currently registered with your professional body with a minimum of 3 years post graduate experience.
• An acute/community therapy background and or experience in Intermediate Care would be highly desirable although not essential.
• A proven record of experience and achievement in assessment, care of adults and older people is essential.
• Knowledge of the chronic long term conditions effecting adults and older people and excellent knowledge of suitable short term interventions to regain independence is a must.
• Evidence of effective leadership management in multi-disciplinary teams and effective liaison with care providers, voluntary and statutory partners is essential.
• Evidence of recent and relevant CPD/Short courses is a requirement and a management qualification or evidence of working at this level is preferred. You will have excellent interpersonal and communication skills, a positive and innovative approach to service development and have a commitment to multi-disciplinary team working/skill sharing. You will also have the ability to support and develop more junior staff and deliver training/provide advice within your specialist area (OT, Physiotherapist, Nurse etc.) to increase a wider skill mix within the team and service. You will also be expected to use all relevant research and audit data to ensure the service continues to meet its requirements and remain evidence based. This role is field based which will include national travel with the flexibility to work from home and requires a willingness to be flexible in approach and attitude. A fully expensed company car will be provided therefore a full driving licence is required.
