Case Manager (Multi-Trauma) job opportunity based in the South of England for one of the largest independence providers of rehabilitation services.

This is a fantastic opportunity for a Physiotherapist/ Occupational Therapist/ Registered Nurse with experience in supporting individuals who have suffered a complex injury or illness.

Role: Rehabilitation Case Manager (Multi-Trauma)
Locations: (Field based and remote working) Swindon, Reading, Bristol, Exeter, Southampton, Brighton, Guildford, Taunton, Maidstone, Cambridge, Basingstoke, Plymouth, Weymouth
Hours: 37.5 per week (Monday to Friday)
Salary: £30,000 - £35,000 per annum (depending on experience) - negotiable for candidates with extensive knowledge & experience

Job Purpose:

As a Rehabilitation Case Manager (RCM) you will carry out specific assessments and pro-active case management on an agreed case load with people who have suffered a complex injury.

Case Manager Duties:

• Facilitate the timely return to work or function of your clients and provide a supportive, personable, professional, and respectful service
• Undertake Initial Needs Assessments and Case Management duties on between 20 -50 cases depending on complexity of case load
• Plan, prepare, co-ordinate and monitor implementation of the rehabilitation programme
• Arrange and coordinate treatment and liaise with existing treatment providers
• Identify the need and make recommendations for other specific assessments including the following; worksite assessments; activities of daily living assessments; vocational assessments; vocational re-direction assessments; functional capacity evaluations and ergonomic assessments
• Undertake specific assessments detailed above or transfer for assessment within the business
• Work in accordance with the Rehabilitation Code 2015, the CMSUK Standards of Practice, the VRA Standards of Practice and any other relevant standards that may be introduced to the rehabilitation market
• Attend and participate in regular team meetings

Case Manager Development & Career Progression:

You will undertake development activities that will expand your skill set and be supported by dedicated managers, trainers and mentors.

You will join a MTD of Multi-Trauma Case Managers which are skilled in physiotherapy, musculoskeletal medicine, psychology, orthopaedics, amputation, brain and spinal injuries, burns and chronic pain.

There are opportunities to progress your career by developing and specialising within a specific area.

Candidate Criteria:

• A Bachelor degree in Allied Health/Nursing/Rehabilitation Science (i.e. physiotherapy, occupational therapy, occupational psychology, exercise science)
• Experience within Case Management
• Physiotherapy/ Occupational Therapy/ Nursing experience
• Full Clean Driving licence
