Central London Community Healthcare (CLCH) working in partnership with NHS West London CCG are looking for Band 8a Senior Nurses to lead in a new model of care designed and developed in collaboration with our patients and health and social care partners. This is an exciting opportunity for Senior Nurses from any nursing discipline to utilise their experience in a newly created role. We need creative thinkers who understand how to adapt to new situations and have a passion to work differently.

The My Care My Way (Whole Systems) model of care is designed to deliver holistic care to frail elderly patients over the age of 65, working in partnership with patients, their carers, the voluntary sector and other health and social care professionals. The service aims to deliver preventative care in the community and primary care settings with a focus on our most vulnerable patients keeping them safe at home and promoting their independence. The team will work collaboratively with patients and their carers to ensure that patients receive the necessary support to understand and manage their condition themselves, wherever possible, with a clear treatment plan and information on the range of services available and how to access them.

In addition to your clinical nursing skills you will also have the opportunity to undertake senior leadership responsibility for a number of Case Managers to support the development of holistic care plans for a case load of older people who have complex health and social care needs.

These roles will be hosted by Central London Community Healthcare NHS Trust (CLCH) on behalf of a group of local providers including Age UK Kensington & Chelsea and Central and North West London NHS Foundation Trust (CNWL) and Adult Social Care.
