Outreach Support Worker
Creative Support is a national, fast growing not-for-profit organisation providing support to individuals with a wide range of support needs. Our services provide responsive and person centred support which is specific to the needs of the individual and the key to achieving this quality of support is by having a staff workforce that is hardworking and committed to ensuring the best possible quality of life and independence for those we provide this support for.
Creative Support’s Personal Budgets are recruiting for Support Workers. The successful applicants will be flexible in their approach and work alongside Social Workers, Occupational Therapists and other health professionals within the wider environment. The aim of the service is to prevent unnecessary hospital admission, supporting timely discharge, maximizing independent living and to assist service users in creating their own unique care package through personalisation.
The key responsibilities of this post are:-
- To hold a personal caseload
- To support service users in their own home to become independent
- Supported service users on a 1:1 basis in their own homes around daily living skills
