Job Description



Ashfield is currently recruiting for a Clinical Educator as part of a new
nurse-led tele-monitoring initiative. The successful candidate will be a
Registered Nurse and will support patients with their chronic disease journey
across Rheumatoid Arthritis and Multiple Sclerosis. The Clinical Educator will
track patients’ progress and will triage alerts based on real-time remote data
to address issues before they become health escalations. They will then be
responsible for acting upon these alerts to change the behaviour of the patient,
ensuring adherence to their personalised care plan, which has been pre-agreed
with their physician.


Key Responsibilities




Patient Care Services (Onboarding, Monitoring, Support and Engagement)
Chronic Disease Management and Coaching:

Based on the data remotely collected from the patient through the digital
platform:

Provide motivational encouragement to help patients stay adherent to their
care plan and treatment goals, attempting to get them back on track should they
become non-adherent
Carry out symptomatic and preventative care interventions to help improve
the condition or wellbeing of a patient, within the clinical governance defined
in the care plans. This includes medical services, as well as non-medical
assistance, supporting the patient in-between visits.
Conduct scripted and unscripted health coaching conversations with patients
taking into account their psychological profile. During those, provide
motivational encouragement and help patients stay adherent to their care plan
and treatment goals, attempting to get them back on track should they become
non-adherent.
Deliver improved care with a broader set of care services, including
validated wellness solutions which will enable the selected provider to reach
out to the Patients and encourage healthy lifestyle behaviors such as exercise,
nutrition, and social matters




Human Triage:

Monitor patient adherence to their agreed care plan, patient reported data,
and signals from the online platform.
Follow a predefined information and escalation plan, qualifying system
signals, providing timely and actionable information to the healthcare
professionals.


Care Coordination:

Be the first point of contact for patients, their family and loved ones, the
extended care team, and other users of the platform.
Facilitate coordination of care and services between the patients extended
care team, service providers, and patient support programmes. Support scheduling
of appointments.


Technical and usage support:

Support all users of the online platform and related specific devices which
technical issues, how-to use, etc., including but not limited to manual and
electromechanical auto injectors, activity trackers, and smart sharps bins.
Coordinate resolution of technical issues, device replacement etc. with
connected service providers.


Support the development of Standard Operating
Procedures



Essential Skills




Registered Nurse as per local requirements
Bachelors/BSN or equivalent work related experience required
Minimum of 3 years’ experience working in MS and/or RA fields is highly
preferred
Ability to access and navigate through CRM systems
Ability to compassionately identify and convey disease state and product
delivery information to patients preferred
Strong interpersonal skills, excellent empathy, emotional intelligence and
an orientation to motivational interviewing towards patient behaviour
change
A self-starter with high personal motivation required
Excellent and effective verbal, written and listening communication skills
required
Proficiency computer operating systems, Microsoft office suite
required
