Main purpose of the post:

The post holder is responsible for the assessment of care needs and the development and/or implementation and evaluation of programmes of care

He/she is expected to carry out all relevant forms of care without direct supervision maintaining a high standard of nursing care within the framework of Trust policies and in accordance to guidelines as laid down by the NMC.

He/she is expected to supervise qualified junior staff and HCAs.

He/she will teach qualified and unqualified staff, including pre registered and post basic students.


RESPONSIBILITIES:

A. Patient Care

Acts at all times in accordance with the NMC Code of Conduct, and within the principles of the scope of Professional Practice and ensures other members of the team also work within these.

Maintain professional working relationships with all members of the cardiac team and effectively contribute to the team's continual development.

Provide nursing assistance to the Medical Team during procedures and investigations

Undertakes the nursing assessment of the patient including risk assessments e.g. Tissue viability, moving and handling and nutrition

5. Devise a plan of nursing care based in the nursing assessment, taking into account the medical prescription

Ensures planned nursing care is delivered safely and effectively taking into account the views of the patient and their relatives. Advise on health promotion and the prevention of illness.

Ensures plans of care for patients are written and updated as appropriate.

Recognise situations that may be detrimental to the health and well being of the individual and take appropriate action.

Works in a team with other nurses liaising with the multidisciplinary team.

8.Maintains absolute confidentiality in respect of all medical and nursing records and also inter-personal issues with colleagues.

Interpret the significance of the patient's clinical observations and records and taking appropriate action but ensuring senior nursing staff are kept informed at all times.

Assist with manual handling and transferring of patients complying with the Trust Moving and Handling policy.

Uses research based practice to provide quality care to patients.

Assist the Unit Manager in maintaining the privacy and dignity issues of patients and staff.

Undertake the management of the care of a group of patients, liaising with medical staff and carrying out their instructions as requested in relation to patient care.


B. Management

Act as unit co-ordinator at the discretion of the ward sister when it is considered beneficial to professional development with available guidance and support from a more senior or experienced nurse.

Co-ordinate the maintenance and safe use of all equipment used on the ward and help to ensure that effective systems are in place for staff training and support with all equipment used.


Liaises appropriately with the Unit Manager ensuring open and effective communication is maintained. In Unit Manager's absence liaises with Head Nurse (Matron).

Reports all sickness to the Unit co-ordinator and/or unit bleep holder.

To have a broad knowledge of and adhere to all Trust policies, procedures and circulars

Promotes a positive unit image by a professional and helpful approach to all team members and visitors to the unit.

Ensures effective communication is maintained throughout all disciplines.

Ensure the ordering, storage and administration of drugs is conducted with strict adherence to national, Trust and NMC policies and guidelines.

Assists the Unit Manager in ensuring that the appropriate an efficient use of supplies is made and exercise economy in the use of consumables.


10.Takes all measures to ensure the safety of the patient - reporting all accidents and incidents, completing relevant documentation.

11.Assists the Unit Manager in providing and developing good nursing practice ensuring that practices are in accordance with department and hospital policy.

12.Assist the Unit Manager Nurse to maintain a safe, clean, pleasant and therapeutic environment.

13.Assist the Unit Manager in ensuring infection control policies and procedures are adhered to and take remedial action when they are not.

14. Participates in Audits/Essence of Care Standards, and any clinical trials undertaken on the unit.

Assists in other wards/departments as requested by the Head Nurse (Matron) or unit bleep holder.


C. Personnel

To act as a role model in professional and personal attributes for all members of staff.

Participates in the induction of new staff and acts as a preceptor.

Effectively works in a team with other nurses. Assists the Unit Manager in the maintenance and development of team morale, is supportive to all colleagues.

Be a visible and approachable resource for patients and staff for clinical and personal issues.

Provide training and guidance to Health Care Assistant staff as necessary.



D. Education

In conjunction with the Unit Manager, assists in maintaining a suitable learning environment for all grades of staff according to the University guidelines.

With direction ensures that student nurses are adequately supervised during their allocation, and reports on their progress to senior nursing staff.

Is conversant with the current curriculae from the University.

Participate in the education of all grades of staff, liaising with the training department and the University.

Acts as a preceptor to students and assess their performance, liaising with the Tutors where necessary.


Maintain and enhance own knowledge through continuing education, training and research activities.

Take part in an individual performance review, reviewing personal development and performance in consultation with the Unit Manager.
