Rhodes Wood
Hospital provides specialist inpatient services unit for children and
adolescents. The social worker role
forms part of our multi-disciplinary team for the delivery of our inpatient
service for young people with learning difficulties and ASD. The service
provides an innovative and psychologically led treatment approach to support
families and young people on managing behaviour that is challenging.

The post
holder will work as part of the core team and contribute to the provision of a comprehensive
high quality multi-component psychology approach across the unit. The post
holder will be expected to provide a social work service as and when required
to young people with in the hospital

The post holder will be responsible for
managing conducting social circumstance assessments, carers assessments and
leading on safeguarding practise within the unit

Key
Responsibilities

Complete
ongoing comprehensive social care assessments and reports as required from the
pre-admission to discharge stage in relation to:



Service
users needs


Risk
assessment and risk management including child protection and welfare issues



Appropriateness
of potential placement incorporating views of family, relevant carers, home
area agencies and those of the victim if available.



Offer a
wide range of social work interventions to both service users, their families
and carers ensuring both their interventions and practice reflects
anti-discriminatory practices.

Contribute
towards the assessment of service users needs under the Care Programme Approach
in collaboration with the responsible LA/HA. In certain exceptional
circumstances act as care co-ordinator and work closely with SSD's in
contributing to a comprehensive assessment of need, section 47 (NHS and
Community Care Act 1990).

In
consultation with MDT and in collaboration with the home LA/HA, contribute to
the preparation of reports for reports for Mental Health Review Tribunals,
appeals to MH Act Independent Hospital Managers and attend hearings if
appropriate.
