Together we can conquer stroke Position: Stroke Recovery Service Coordinator Location: Manchester Job Type: Fixed Term (Their services are contracted, they currently have funding for this contract until 31st March 2018) Hours: 25 per week Salary: £15,997.97 to £17,517.84 Closing Date: 17th October 2017 Interview Dates: 26th October 2017 About the role: They are looking for an enthusiastic and motivated individual to join their team in Central Manchester. Working closely with the hospital and community multi-disciplinary teams you will provide emotional and practical support to stroke survivors, their families and carers. Reporting to the Regional Management Team, the Stroke Recovery Service Coordinator will provide support and information to stroke survivors and their families both on the ward and at home. By assessing the needs of each stroke survivor and their family you will aim to reduce the risk of further strokes by supporting people to make lifestyle changes based on their identified risk factors, along with helping them to set personal goals and work towards them. The service operates within Central Manchester with referrals being received from the Community Stroke Team, families and other healthcare professionals. About you: Applicants are likely to have a background in the caring profession, preferably with knowledge of stroke. You must have all round flexible skills in listening, communication, organisation, IT and resourcefulness. The role will require you to have your own car and a current driving licence.
