We currently have an exciting new opportunity to join our expanding Outreach service as an Outreach Support Worker. Role: Outreach Support Worker Location: Watford & Surrounding Area Salary: £16,500 per annum (pro rata for part time) Company: Life Opportunities Trust Job Type: Full or Part Time - Flexible Rota over 7 days Job Reference: OR37SW Our Outreach Team provide support for clients to be as independent as possible within the community.  Clients may have learning disabilities and/or physical disabilities, or may just need extra support.  Typical duties involve assisting clients to complete their shopping, attending social and leisure activities, and assisting with maintaining their health and welfare.  The role also can involve some personal care, medication, preparation of meals and domestic duties. Previous experience is not essential however you must hold a current driving license, and be willing to use your own car. Successful candidates will undergo an enhanced DBS disclosure.  In return, LOT offers an extensive training and development package, contributory pension scheme, and an employee assistance programme.  Key responsibilities: To support nominated clients in their own homes and within the community. To encourage clients to participate in all aspects of daily living activities. Assist the management to maintain good relationships with other personnel involved in the care of the client, i.e. Community Nurses, Social Workers, G.P’s, etc., as part of a caring team. To carry out all tasks in a way that demonstrates respect for the privacy, dignity and value of all clients, irrespective of severity or disability or personal circumstances. To assist clients with personal care. To assist with domestic duties. Maintain accurate, legible records and submit them on a regular basis to the manager. To complete, update and participate as appropriate in client’s assessments in conjunction with other team members and relevant professionals, family and friends. Maintain confidentiality at all times. Advise clients of the complaints procedures (where appropriate). Comply with legal requirements under Domiciliary Care Agency Regulations, CQC and Health and Social Care Act. To act on own initiative and respond responsibly and effectively to the demands of a regular changing client group. To attend regular staff team meetings and staff training days as required. The successful candidate will be able to undertake admin tasks; communicate effectively and work shift work including evening, night and weekend duties and client holidays as required. Outreach Support Workers are also expected to work towards an NVQ Level 2 in Health and Social Care if they have not already got this qualification.
