The service will work towards ensuring clients at risk of rough sleeping, are brought off the streets, as well as provide an assessment hotline for referrals and triage. The new service will also have lead workers in all boroughs across West London, assessing NFNO clients, sustaining clients off the streets and enabling them to reach their life goals.

You will be responsible for delivering, a rapid assessment service to assess people at risk of rough sleeping; either by phone or email. You will do this in partnership with your colleagues, day centres, outreach teams, local authority and other partners.
