Main Purposes of the Job:
Improve hospital discharges for care homes resident’s across Hertfordshire hospitals. Engage across hospitals teams, care homes and social care to build relationships and trust. Facilitate and share learning to improve discharge services. Assist care homes to provide high quality care by supplying detailed assessments to the care home and support for any changes in need.

Contract Terms:
Numerous options available across Princess Alexandra Hospital-Harlow and Watford General
For example:
Monday to Friday- 37.5 hours (8 hours a day)
Saturday/ Sunday- (8 hours a day)
Three Day working week- eg. Monday- Wednesday or Thursday to Saturday

Bank Holidays may need to be covered on occasion and will be paid double time
On occasion staff will be requested to work across another Hertfordshire Hospitals or on different days

Specific Responsibilities:
1. Work within the hospital discharge team to support and facilitate timely and safe discharges from hospital to care homes

2. Complete an assessment on behalf of care homes with new and current residents to facilitate their discharge.

3. Liaise with the hospital wards to ensure a smooth discharge for the resident and care home

4. Liase with care homes around the current discharge situations for their residents

5. Act as a point of contact when residents are admitted to hospital from care settings to monitor progress

6. Work in partnership with care settings and the hospital to find solutions to current concerns

7. Work with current systems to support the discharge process

8. Where required provide data and reports on the service

9. Liaising with families when necessary to keep them informed

10. Visiting care homes to create and continue relationships

Person Specification:
• Must have current RGN PIN
• Should have experience or understanding in the workings within care homes
• Should have experience or understanding in the day to day workings of hospital
• Excellent communication skills both written and verbal
• Has passion and enthusiasm to raise the standard of quality of care
• Has the ability to build rapport and negotiate
• Willing to learn new skills and to develop current knowledge
• Excellent time keeping
• Must have own transport with insurance for business use is essential for this role as travel around Hertfordshire may be required
