About the Role

We are currently seeking three Support Coordinators to join our Supported Housing team in Redhill, Surrey. The main purpose of the Support Coordinator role is to deliver support to customers in our sheltered housing schemes by assisting with key areas of their lives, acting as a central point of contact, and working to ensure that customers receive all the services that they require.
Your main responsibilities will include assessing new residents to determine which level of support would most appropriately meet their needs; developing and reviewing support plans and risk assessments; undertaking regular contact with customers including visits, telephone calls, emails and text messaging; responding to emergency calls; and proactively promoting and encouraging independence.
The successful candidates will have worked within a similar role previously and will ideally hold a relevant qualification. You will have experience of person-centered support planning, an awareness of the high profile issues relating to vulnerable / older people, along with a basic knowledge of assistive technology and adaptations. We are seeking strong communicators who have an ability to deal with challenging situations, and can ideally demonstrate some knowledge of “coaching” and “motivational interviewing” when working with customers to identify support needs.
A full UK driving license and access to a vehicle for work purposes is essential.
