Job Details

£32,839 per annum for 36 hours per week based at Fairmount House, Bull Hill, Leatherhead, Surrey KT22 7AH.

2 year Fixed Term Contract which is linked to specific project work. This is a full-time position but Part-time applications will be considered.

About the Role

Are you a qualified, HCPC registered social worker who has successfully completed their Assessed and Supported Year in Employment (ASYE). (NB this applies only to social workers who qualified on or after 2012).

Skills required:

You will need to be motivated and flexible with high professional standards; having sound knowledge and experience of current legislation, including the Care Act, NHS Continuing HealthCare and the Mental Capacity Act and their application to young people in transition to adult services. You will need to be confident in your skills to work with individuals, their families and in multi-disciplinary settings, including educational establishments and in partnership with NHS colleagues.

You will need to be confident in screening for NHS Continuing Healthcare, applying and articulating Care Act eligibility criteria within the context of multi-disciplinary assessments, and working with a diverse group of individuals and partner agencies to obtain relevant assessments to understand presenting care needs.

You will be able to communicate in a clear, fluent, concise and jargon-free way, remaining courteous, calm and professional, even when individuals and other professionals are reluctant to engage. You will need to be confident in holding challenging conversations and able to explain your professional decision making / rationale for social care eligibility within written reports and NHS decision making processes.  

You will be able to supervise less experienced members of staff within the team and support their development. You will need to provide opportunities for less experienced staff to shadow your practice, using coaching skills to develop their practice. How we will support you in your role:

  • You will attend a full 3 week induction where you will undertake all the mandatory training you will need. This will include classroom training, e-learning and a range of work place shadowing opportunities.
  • You will receive an annual appraisal along with regular supervision sessions
  • You will be assigned a “buddy” to help you settle into your new role
  • Be provided with a Practice, Performance and Personal Development folder to support your career development
  • Have access to a wide range of training opportunities to help you develop yourself within the role
