Salary £20,124 rising to £20,760 per annum after 1 year

Join a brand new team delivering exciting community based support service in London Borough Hounslow!

Commencing in January 2017, the Hounslow LIFE Integrated Support Service is an exciting new project that supports adults at risk of losing their independence through the delivery of personalised preventative support in the community.

This is a great opportunity to work in a brand new team on this high profile new contract in Hounslow, working with a range of people at risk of losing their independence to identify their strengths, goals and aspirations and build their wellbeing and empowerment. In addition to more traditional support work, you will be facilitating group activities, developing peer support and other community based networks.

If you have any convictions/cautions, this does not mean you cannot apply for any of our clients’ roles, however they do require a DBS for all positions. They will ensure that any positive disclosures on DBS records are discussed with candidates and fairly assessed by an internal panel.

Person specification:

• Ability to work with people who have a range of long term conditions, health and complex needs in a person-centred way
• Ability and willingness to lone work
• Ability and willingness to work remotely and in the community, using mobile technology
• Excellent communication skills and attention to detail

Hours: 39 hours per week
Working Pattern: The service operates Mon - Fri from 8am - 9pm and Sat - Sun from 10am - 5pm so some flexi working to ensure contract is delivered within these times will be required.
