Thornbury Community Services is currently looking to recruit a knowledgeable regional clinical lead to work with our complex care packages in the Bristol Region and surrounding commutable areas (Bath, Gloucester, Oxfordshire, Swindon and Wiltshire etc). TCS are a well established provider of complex nursing care in the community placing registered nurses into packages, caring for children and adults living at home. Our trusted nurses are responsible for the day to day care of the client and will provide clinical support to the family.

Reporting to the Divisional Clinical Lead, the Regional Clinical Lead is responsible for providing service excellence through the development and growth of the Community Care Team. They are responsible for ensuring all KPIs are met whilst developing structure and strategy to best deliver results.

Key Tasks
• Responsible for undertaking initial holistic nursing assessments in partnership with the Service User/family/carer and involving other statutory/voluntary/third 'party’ sector providers as necessary
• Responsible for developing individualised Care Plans in partnership with the Service User/family/other Health Care Professionals as appropriate ensuring goals are patient focused, realistic and achievable.
• Provide On Call support to the Community Care team on a roster basis.

• To ensure high standards and excellence in clinical practice within area of responsibility, reflecting current organisational and national guidelines and standards.
• Undertake clinical work to maintain and develop clinical skills
• Assist with training of community staff for specific care packages
• To work with Multi-Disciplinary Teams in the management of care and appropriate response to changes in Service User needs
• To be aware of the potential for vulnerability and abuse amongst the service users/families and use local policy to inform appropriate personnel in line with Safeguarding Vulnerable Adults and TCS / Group Policies

Professional / Personal Development
• To adhere to The Nursing and Midwifery Code; Standards of conduct, performance and ethics for nurses and midwives
• To plan, deliver and evaluate programmes of education for HCSW for each package of care as appropriate
• To undertake self-directed study maintaining knowledge of national strategies impacting upon the Health of Service User
