Band 6 Community Psychiatric Nurse, Community Rehab Team

Sanctuary Health is currently looking for an experienced and dedicated community psychiatric nurse to work within a community rehab team in Westminster. The ideal candidate will possess highly developed specialist knowledge of community psychiatric nursing and be registered with the NMC. You will be required to work 30 hours per week.

The pay rate for this locum CPN job is £27.33 (LTD Company Equivalent) per hour and has an ongoing contract.

Community psychiatric nurse duties include:
- Working proactively with provider services to draw up resettlement plans and carry out regular reviews of their progress towards identified goals
- Providing comprehensive, culturally appropriate, evidence based assessments to establish eligibility for health and social care services and consult/refer to other services when appropriate
- Ensuring a financial assessment has been undertaken when required for accessing residential care or funded social care package

Requirements of the community psychiatric nurse:
- Degree or equivalent in mental health nursing
- Current NMC registration
- Significant NHS experience as a community psychiatric nurse
