BS Social Care specialise in the permanent recruitment of Health & Social Care professionals. We are currently assisting a leading nursing and residential Care provider in their recruitment of an experienced Team Co-ordinator.

Our client is known for their personal service and require dedicated and driven individuals to help the business to continue to grow from strength to strength. As Team Co-ordinator you will play a key role within the service and be responsible for supporting the residents and staff alike on a daily basis.

Other duties will include:

* Undertake specified, managerial responsibilities, which contribute to the overall management of the support services provided.
* Be involved in the assessment of service users and potential service users.
* Implement care procedures and plans which support the service user in a manner which respects their dignity and promotes development of their independence and quality of life.
* Liaise with families, social workers and other professionals on behalf of the service user and the service and participate in case conference/reviews.
* Work within a rota which may include day, evening and weekends and sleep-in duties.

An excellent communicator, you will truly be passionate about making a difference to the lives of the people you support.

To be successful in this role you will meet the following:

- Minimum 2 years experience in a similar management role supporting adults with learning disabilities, autism and challenging behaviour

- NVQ Level 3 or above in Health & Social Care or Management

- Excellent communicator with proven experience building, and maintaining relationships at all levels

- Driver with own transport is essential
