About the job:
The scheme is based in Camberwell and provides support to customers with mental health needs from a forensic background. This team work closely with South London and Maudsley clinical team to provide support to customers who live at the service. You will work flexibly to ensure individual needs are met while maximising independence, choice, dignity, privacy and well-being.

Main accountabilities:
-Advising customers on all aspects of tenancy support including accessing benefits and budgeting and accessing appropriate services, including some housing management.
-Report any safeguarding or ASB issues
-Carrying out health and safety checks
-Reporting repairs and encouragement to undertake meaningful activities within the local community as well as attending appointments with customers.
-Work with customers to develop, review and update of care and support plans and risk assessments in line with policy.
-Encourage and support customers to establish and maintain positive relationships with their family and friends and engage in activities which help them to achieve their personal goals.
-Be aware of customers` wellbeing, safety and state of health, reporting any concerns.
-Ensure all necessary records are accurate and up to date.

About you:
-Experience of providing support to people with mental health needs
-Experience of support planning and risk assessments
-Knowledge of health & safety issues relating to care and support of vulnerable people
-Availability for working unsocial hours, weekends and public holidays on a rota basis
-On-call service and sleep ins where appropriate
-Ability to work using own initiative and as a member of a team
-Level 3 Diploma in Health and Social Care related subject area or equivalent, or a willingness to undertake Diploma training
-Knowledge and understanding of relevant legislation
