Job Purpose
This post will support organisations who are members of the Special Educational Needs (SEND) Health Group to review current processes in respect of health input into the development and/or review of Education Health and Care Plans (EHCPs). It will work to devise processes that support and maximise health’s input to the production of EHCPs, utilising existing resources and supporting providers to transition from the current state to a new arrangement.

This post will contribute to the delivery of the Children’s programme priorities and support the achievement of the vision and strategic plans of the health economy, the priorities of the Strategic Partnership for Children and Young People with SEND and Behaviour and the broader West Yorkshire and Harrogate Sustainability and Transformation Plan (STP).

Main Job Responsibilities

The post-holder will engage with providers of health services for children and young people across the Bradford district and Craven, education and local authority colleagues to devise a process which supports health to respond to its Statutory requirements in respect of the provision of health advice to EHC Plans.

The post-holder will be accountable to and receive support from members of the Special Educational Needs (SEND) Health Group and the organisations members represent.

Being located within the Strategic Partnership Team will allow the post holder to work as part of a dynamic group including service users, patients, clinicians and colleagues (internal and external) to deliver the objectives and outcomes of this system-wide transformation project.

The post holder will:
• Support the Head of Commissioning in ensuring that the health organisations are able to respond to its Statutory requirements in respect of the provision of health advice to EHC Plans
• Ensure this project is appropriately linked with the broader SEND programme of work being delivered by the Strategic Partnership for Children and Young People with SEND and Behaviour
• Lead the development of short, medium and long term plans required to achieve the transformation of health and care services required to deliver this project
• Identify interdependencies across functions, potential impacts on wider organisation resource requirements and build in contingency and adjustments as necessary
• Ensure close partnership working with clinical board leads, clinical specialty leads, CCG teams, health and care commissioners and providers, education and local authority colleagues to ensure effective patient pathways and services are in place
• Develop and maintain project documentation where relevant, contributing to a coherent management system
• Manage the monitoring of project implementation ensuring that each project within programmes has clearly identified milestones and outputs
• Report on the progress of each activity against key milestones, identifying slippage and liaising with work stream and other leads to ensure that mitigating action is taken as required bringing the project back on trajectory
• Liaise with stakeholders, as appropriate, including patients and carers, commissioners, providers, education and local authority colleagues
• Work with relevant individuals and organisations to investigate the causes of any variance from target/plan and proactively contribute to the implementation of solutions.
• Establish information requirements and reporting mechanisms to monitor and track performance and to satisfy internal and external reporting requirements
• Write and implement corrective action plans, where necessary
• The post-holder will seek examples of good practice from other areas and make recommendations in regard of future service provision
The post-holder will be flexible and able to respond to changing priorities.
