Thornbury Community Services is currently looking to recruit a knowledgeable case manager to work with our complex care packages in Northern England. TCS part of the ICS group are a well-established provider of complex nursing care in the community placing registered nurses into packages, caring for children and adults living at home.

Community is at the heart of everything we do therefore your role of Case Manager will be supporting not only the packages but also our staff working within them. TCS offers you all the support you need through our dedicated teams throughout the business.

Reporting to the Business Manager and Senior Case Manager, the Case Manager is responsible for providing quality community care service to stakeholders which include clients, families, Commissioners, Regional Clinical Leads and any others where required. The Case Manager’s role is to assist individuals to live within their own home and communities, whatever their abilities.

Key Tasks
To be the sole point of contact for the client and family in all aspects of care management
Liaise with the commissioning bodies, Regional Clinical Leads, Business Manager and others within the healthcare sector to ensure the success of the care
Confidently liaise with all relevant health professionals, social workers, families and clients
Keep abreast of current commissioning developments regarding the community healthcare sector
Responsibility for a case-load of packages of care, and develop from the initial assessment with the Regional Clinical Lead.
Review and monitor care packages in conjunction with the Regional Clinical Lead
Willing to travel and undertake client visits
Work with the Multi Disciplinary Team at all levels both internal and external

Skills Required
Full UK Driving Licence
Professional Qualification
Working knowledge of Risk and Clinical Governance
Working knowledge of Safeguarding and DOLS
Ability to manage difficult situations and provide professional challenge
Committed to promoting the health and social care industry
Ability to prioritise, plan and execute plans, processes and tasks effectively
