Community Clinical Case Manager – SCP Complex Care

SCP Complex Care requires a highly motivated, experienced nurse of management level to undertake the role of Clinical Case Manager within the Complex Care Department. SCP Complex Care is dedicated to improving the lives of individuals they care for and ensure that they promote person centred practice and individual choice.

This post is an opportunity to lend your skills and experience to a community based service that provides valuable care to both adults and children that have specific and/or complex needs by managing the care of our clients and aiding the training of our workforce.

We are looking for an individual with management experience, who can work autonomously, manage their own workload and pass on their enthusiasm and passion for providing person centred care to others.


  • Manage own case-load of clients who have complex needs such as gastrostomy, epilepsy, acquired brain injury, tracheostomy, ventilation

  • Co-ordinate of packages of care

  • Ensure care plans are written and accurate as per the client’s needs and wishes

  • Ensure all candidates have regular reviews so that the care plan and delivery of care is appropriate and to make any changes that are needed to care plans

  • Ensure all carers have been trained in specific skills relating to their clients and that they have been deemed as competent to undertake care such as enteral feeding, epilepsy care, tracheostomy care

  • Support growth of the department by working alongside the Head of Complex Care

  • Attend MDT meetings and update other professionals with information

  • Evaluate the safety, effectiveness and sustainability of care packages and escalate and investigate any concerns

  • Ensure the safe delivery of care to all clients and ensure all clients are receiving the best care with highly skilled carers

The role will be split with the majority of your time spent in the community, covering clients (both children and adults) within the Essex region. You will also be required to deliver training at our Maidstone branch for at least 4 days per month on the essential skills for carers.

Person Specification

The candidate must have the following:

  • Valid NMC PIN number in Adults or Children’s nursing

  • Management experience

  • Experience in community nursing or intensive care nursing

  • Experience with complex care such as enteral feeding, tracheostomy, ventilation, epilepsy, ABI

  • Excellent communication skills

  • I.T. skills
