The role of the Respiratory Specialist (Nurse or Physiotherapist) is to improve patient outcomes.

Band 6, 26 hours per week. Mileage paid. 

Your principal responsibilities will be :-

To ensure the care patients receive best meets their needs whether through pulmonary rehabilitation and oxygen therapy.

Provide evidence based Pulmonary Rehabilitation to patients by:
• Identifying and recruiting suitable patients to the programme
• Conducting initial and final discharge assessments, recording baseline results and goals at the initial assessment and following the programme recording the outcomes for each patient
• Leading Pulmonary Rehabilitation supervised exercise sessions
• Presenting education sessions on a range of topics including understanding the disease, self management, medicine management, managing breathlessness
• Providing discharge reports to the patient and the patients GP

Provide evidence based Home Oxygen Assessment and Review by:
• Assess existing home oxygen patients and all new patients referred for assessment
• Pre-screen patients to ensure that only those with a clinical need are invited for oxygen assessment, and to ensure that all relevant patient information is provided by the referrer prior to the first appointment
• Undertake capillary blood gas sampling and accurately interpret the results
• Conduct 6 minute walk tests or shuttle walk tests with patients requiring ambulatory oxygen therapy
• For those patients who are not hypoxic and therefore do not require home oxygen therapy, withdraw home oxygen therapy. Where patients are anxious, follow the oxygen removal pathway to aid the process by gradual removal supported by education.
• Where home oxygen therapy is clinically required, conduct a follow up home visit at 6-12 weeks following assessment and 6 monthly oxygen reviews
• Educate patients and their carers on the use of home oxygen therapy including the risks associated with incorrect use and self management of their condition including smoking cessation and nutritional advice
• Accurately and promptly complete the HOOF and forward to the Home Oxygen Supplier

To ensure integration with other local Health Care teams and professionals by:
• Ensuring onward referral to other teams / services in line with local referral protocols
• Maintain appropriate and current patient records
• Liaise with patient families and their carers and manage emotional situations professionally
• Review complex cases with Healthcare Professionals as appropriate
• Becoming a source of respiratory expertise to all health care professionals and provide specialist respiratory training, advice and support to all health care professionals who require it.

You should be flexible to work across both parts of the services: Pulmonary Rehabilitation, Home Oxygen Assessment & Review, as required to meet local 

Required key skills my client will look for:-

• Experience of respiratory care including at least one of the following;
o pulmonary rehabilitation
o home oxygen assessment
• Experience of arterial/capillary blood gas sampling, measurement or interpretation
• Experience/Awareness of walk tests within respiratory care
• Experience of conducting spirometry and interpretation
• Confident in using computer and electronic patient management software
• Highly motivated and capable of self managing
• Ability to demonstrate knowledge and understanding of home oxygen guidelines.
